1. Authorization
By submitting the intake form at /community/connect, you (the operator or an authorized representative of the community) authorize Healthcare.Engineer LLC to:
- Verify your facility’s license information against public AHCA records (or the equivalent in other states).
- Contact you for follow-up regarding your tier interest and a custom package proposal.
2. Accuracy certification
You certify that the information you submit — facility name, state license number, bed count, services, specialty endorsements, ring-of-care partners, multi-facility ownership flag, and tier interest — is accurate to your knowledge at the time of submission.
3. Custom-package process
You understand that pricing on H.E 360 is custom and quote-only for community-side tiers. After we review your intake, we will propose a tailored package via Stripe Invoicing within approximately 48 hours. You may decline the package. No obligation exists until you pay the invoice.
4. Communication
You authorize transactional emails from contact@healthcare.engineer regarding your intake. You may opt out of marketing communications at any time without affecting transactional emails.
5. Data scope
Intake data is used to assess your tier, propose your package, and (after invoice payment) populate your listing. See our Privacy Policy for our full data & privacy posture.
6. Governing Law
These Terms are governed by the laws of the State of Florida. Disputes shall be resolved in the state or federal courts located in Orange County, Florida.
