1. Not Legal, Medical, or Compliance Advice
HE360° products and services do not constitute legal advice, medical advice, healthcare consulting, regulatory compliance consulting, or any other form of professional advice. Nothing on this platform should be interpreted as such.
Our Fix-It Services provide data-driven observations and informational recommendations based on public inspection records. These recommendations are not compliance directives. ALF owners should consult with licensed attorneys, compliance consultants, and regulatory professionals before making operational or policy changes.
Our Matching Services help families organize publicly available information. They do not constitute medical referrals or clinical assessments of resident needs. Families should work with healthcare providers to determine appropriate care levels.
2. Data Accuracy & Limitations
While we strive for accuracy, users should understand the inherent limitations of our data:
- Source dependency: Our data comes from publicly available state health department records. We do not control the accuracy, completeness, or timeliness of government-published data.
- Point-in-time snapshots: Inspection records reflect conditions observed at the time of inspection. Facility conditions may have changed since the most recent inspection.
- Processing delays: There may be a delay between when a new inspection is published by a state agency and when our scores reflect that data.
- Scoring methodology: HE360° scores are generated by our proprietary algorithm. They represent our analytical interpretation of public data and are not official government ratings. Different methodologies could yield different results.
- No guarantee of completeness: While we aim to cover all facilities, there may be newly licensed facilities or recently closed facilities not yet reflected in our system.
Always verify information directly. Before making any decision about an assisted living facility — whether as an owner or a family member — verify all information directly with the facility, the relevant state agency, and other authoritative sources.
3. Not a Licensed Referral Agency
Healthcare.Engineer LLC and the HE360° Intelligence Platform do not operate as a licensed senior placement agency or referral service under Florida Statute 400.474 or any equivalent state regulation.
Our Matching Services are informational tools that cross-reference user-provided preferences with publicly available data. We do not:
- Receive referral fees, commissions, or any form of compensation from assisted living facilities
- Place or arrange the placement of individuals in facilities
- Advocate for or recommend specific facilities over others based on business relationships
- Act as an intermediary between families and facilities
All matching results are generated algorithmically based on public inspection data and user-provided criteria. The decision of where to live is entirely the user's.
4. Scoring & Assessment Disclaimers
4.1 HE360° Scores
HE360° scores (0-100 scale) and letter grades (A-F) are proprietary analytical products. They are:
- Not government ratings, certifications, or endorsements
- Not predictive of future facility performance or safety
- Based solely on historical public inspection data, not on unreported conditions
- Subject to change as new inspection data becomes available
4.2 Assessment Language
When our products use terms like "Strong Record," "Mixed Record," "Concerns Noted," or "Significant Concerns," these reflect our algorithmic assessment of publicly available inspection history. They describe what the public record shows — they are not verdicts about a facility's current safety or quality of care.
4.3 Quadrant Placement
Our quadrant system (Verified, Caution, At Risk, Critical) categorizes facilities based on inspection data patterns. These categories are analytical classifications, not safety guarantees or warnings. A "Verified" facility may still have issues not captured in inspection records, and a facility categorized as "Critical" may have already remediated its deficiencies.
5. "Reality Gap" Disclaimer
The "Reality Gap" compares a facility's Google reviews/ratings with its public inspection record as analyzed by HE360°. This comparison is provided for informational context only. Google ratings reflect subjective user experiences, while inspection records reflect regulatory observations. Neither alone provides a complete picture of a facility. Discrepancies between these data points do not necessarily indicate deception or wrongdoing by any party.
6. Fix-It Services Disclaimers
Score projections are estimates, not guarantees. When we project that fixing certain deficiencies may improve a facility's HE360° score, these projections are based on our scoring model and historical patterns. Actual score changes depend on future inspection outcomes, which we cannot control or predict.
- Implementation of our recommendations does not guarantee regulatory compliance, passing future inspections, or avoiding penalties
- Our staff are data analysts, not licensed compliance officers, attorneys, or healthcare administrators
- Policy template recommendations are informational starting points, not legally vetted documents — have them reviewed by a qualified attorney
- Staff training topic recommendations are based on deficiency patterns, not clinical assessments — consult qualified trainers for implementation
7. Matching Services Disclaimers
- Facility matches are based on publicly available data and user-provided preferences — they are not clinical assessments of appropriateness
- Cost estimates are approximate and based on available public or published pricing data — actual costs may vary and should be confirmed directly with facilities
- Insurance and Medicaid compatibility information is based on publicly available data and may not reflect current facility acceptance policies — always verify directly
- Our matching does not account for factors not captured in public records, such as staff personality, facility atmosphere, food quality, or activity programming
- We strongly recommend visiting all matched facilities in person before making any placement decision
8. "HE360 Verified Improvement" Badge
The "HE360 Verified Improvement" badge indicates that a facility's HE360° score improved during participation in our Premium Fix + Implement program. This badge:
- Is not a government certification, accreditation, or endorsement of any kind
- Reflects improvement in our proprietary scoring system only
- Does not guarantee ongoing compliance, safety, or quality of care
- Is subject to revocation at any time if the facility's HE360° score declines based on subsequent inspection data
- Should not be represented as a government-issued or industry-standard certification
